Please read this email entirely before setting up your screening appointment.

The Kg. screening will be held on Thurs 4/23 or Friday 4/24 in the elementary gymnasium which is located through the Jackson Street entrance. The parent(s) will sit on the stage in the gym (in view of your child) while your child meets with a kindergarten teacher at a table set up on the gym floor. We will have you complete some paperwork for our records while your child is being screened. After the teacher is done with the screening, they will call you down and go over their assessment. Your child will also have their picture taken and they will receive a free t-shirt and coloring page during their screening. After you meet with the teacher, your child will see the school nurse Mrs. Caryn Clinger for a vision screening and Mrs. Angie Brown the speech therapist for a hearing screening.

You will receive another email once you enter your information at the provided link below. You will then select a day and a time for your child's conference either on Thursday, April 23 or Friday April 24. DO NOT select more than one date or time. If you accidently do this, we will delete one of the dates/time from our schedule. Please DO NOT share this link with your friends. The parents who have COMPLETELY enrolled their child in the 2026/27 school year in Final Forms will have this link emailed to them.

The deadline to schedule the screening online is Monday, 4/20. If you miss the deadline OR have problems scheduling a time, please call the office at 419-358-7951 and Julie Oberly will schedule it for you.

Allow yourself at least an hour of your time for your scheduled appointment.
